Payment Plan request for Sandy Shimoda 2020

Below is a request form to setup a payment plan for ticket purchase to our Sandy Shimoda event over 5 & 6 December 2020. The total amount payable for this ticket item is $900, which will be deducted across 15 fortnightly installments of $60 each. By filling out the below form you give your permission to Proper Pilates to setup a payment plan schedule and begin deductions from your account linked to your Proper Pilates Mindbody profile. If you’re a new client to PP please fill in this form and we will be in contact with you to setup your Mindbody profile.

After completing this form you don’t need to do anything else. You’ll receive a payment receipt from us once everything is set up, or a phone call if we need more information. Remember it’s your responsibility to ensure you have adequete funds in your account to meet each scheduled deduction. If overdrawn and we can’t take the payment, you will be responsible for reimbursement of associated bank charges to Proper Pilates.

No refunds or exchanges will be available for this ticket option. Should you decide you no longer wish to attend, Proper Pilates will not offer refund for any monies paid to date.
